Housing Stabilization Services

WHAT IS HSS?

Housing Stabilization Services is a housing service for all eligible Minnesotans created by the Department of Human Services. The service is designed to help people living with disabilities to have the person-centered assistance they need to be successful in their housing endeavors whether that be moving across the street, across the state, or even finding resources to help prevent homelessness.

Transition vs Sustaining

Brilliant Minds is thrilled to present a distinctive strategy to Transition and Sustaining services that will help the people we serve in our community to reach their potential and meet their housing related needs.

Transition will help with:

  • Creating a housing services plan
  • Finding independent housing in the community
  • Navigating housing applications, tours, and the lease signing process
  • Searching for community resources to arrange payment
  • for fees and deposits
  • Assistance with packing and unpacking household items
  • as well as move-day support

Sustaining will help with:

  • Creating an individualized, housing related goal plan
  • Meeting in-person each week to
  • work on housing goals
  • Advocacy to housing supports

Goals

Who can receive services?

You may qualify if you:

  • Have medical assistance
  • Are 18 and older
  • Have a disability or disabling condition
  • Are homeless or at risk of homelessness
  • Have a qualifying assessment

What can Housing Stabilization do for you?

  • Help you plan for, find, and move into housing
  • Help you keep housing after moving in
  • Provide assistance finding financial resources
  • Communicate with landlords
